About the service
What is geriatric conditioning?
Geriatric conditioning is physiotherapy designed specifically for older adults. As we age, changes in muscle strength, balance and coordination can increase the risk of falls and reduce confidence in everyday activities. Our physiotherapists work with older adults and their families to identify risk factors and develop practical, achievable programs that fit into daily life.
We focus on maintaining mobility, strength and balance — helping people stay active, independent and confident in their own home and community for as long as possible. We also support recovery from surgery, illness, or periods of reduced activity.
We offer home visits so we can assess your loved one's environment and tailor recommendations to their actual living space — making therapy as practical and relevant as possible.

Our approach
How we support older adults and their families
We understand that getting older can bring new challenges — and that families often play a central role in supporting their loved ones. Our approach is practical, respectful, and focused on what matters most to you.
Falls prevention
We assess fall risk factors including strength, balance, gait, medications, and home environment. We then develop a targeted program to reduce risk and build confidence.
Strength and balance
Evidence-based exercise programs to build the strength and balance needed for safe, independent movement — tailored to current fitness level and goals.
Post-surgical recovery
We support recovery following hip and knee replacements, fracture repairs, and other procedures — helping people regain mobility and return to their normal activities.
Home environment assessment
We visit the home to identify hazards and make practical recommendations — from furniture placement to equipment such as grab rails, shower chairs, and walking aids.
Carer education
We provide guidance to family members and carers on safe handling, transfer techniques, and how to support their loved one's therapy goals at home.
Aged care coordination
We work with aged care providers, GPs, and other allied health professionals to ensure a coordinated approach to your loved one's care.

Referring a patient
Make a referral
We welcome referrals from GPs, geriatricians, aged care providers, and families. Getting in touch is straightforward.
Get in touch
Email or call us with the patient's name, current concerns, and any relevant medical history. No formal referral form is required.
We will follow up
Our team will contact the patient or their family directly to discuss their needs and arrange an initial assessment at a time and location that suits them.
Ongoing communication
With consent, we will keep you updated on progress and can provide written reports for aged care reviews, GP management plans, or NDIS plan reviews.
